What does this mean for me?

We will make all efforts to assist you in receiving uninterrupted care. We have listed some options for your future dental care:

Transfer service: If you retain your Medicaid coverage now and in the future, the Dentaquest provider directory is a great resource to find a new dental office that is accepting new patients with Medicaid: https://dentaquest.com/state-plans/regions/colorado/find-a-provider/

We will help you transfer your records to your new office. Just submit the record transfer form on our website, which can be found at: https://applegrovedental.com/record-release-form/

New dental coverage: There may be options for you to change dental insurance coverage now or in the future. We continue to accept a wide range of insurances. A complete list can be found at https://applegrovedental.com/payment-insurance/

If you obtain new coverage, we would be happy to help you learn more about your new coverage.

Self-pay discounts: If you no longer have Medicaid coverage now or in the future, we are happy to extend our pricing for self-pay patients without any insurance coverage. We also offer multiple convenient options for self pay patients, including credit card payment, Care Credit, and cash discounts.

We are grateful for your trust in our office and in the care that we provide. Please do not hesitate to call us with any questions or concerns.
